I take appointments
Tues, Thurs, and Fri
9am to 5pm but
am flexible about arranging other times.

I can meet with you in my office using appropriate Covid protocols. If you prefer, we can meet by phone, FaceTime, or Zoom.

You can download an Intake Form prior to your appointment. Mail it, email it, or bring it to your appointment.

Fees

Intake session is $175
Subsequent sessions are $165
Payment is expected at time of service unless otherwise arranged.
I accept personal checks and debit/credit cards but require that you pay the 3% service charge.

I am covered by most insurance companies but please check with your plan to make sure I am a preferred provider. Please note that Medicaid and Medicare do not reimburse my services. Also check to ensure that teletherapy is covered if we meet remotely.

Policies

I suggest you meet with your primary care provider for a physical checkup prior to our appointment. Depression and anxiety can be caused by physical problems, such as thyroid issues.

I require 24 hour notice for cancellations, unless ill. You will be charged for late cancellations as they cannot be billed to insurance companies.
